(PDF) Online Form Instructions
Forms on this page can either be printed and filled in by
hand or completed online then printed. To complete the form
online follow these instructions:
| 1. To type in a form, first click the hand button on the top toolbar to make sure you are in data entry "mode." Next, click in a form field (blank space where text is required) and begin typing. |

| 2. To mark a check box, simply click in it; click again to un-mark it. |

| 3. When you have completed filling in the form, click the Save button to save the file to your computer. |

| 4. Print the file by clicking the Print button. |
